Healthy hair and skin relies largely on vitamin B7, also known as Biotin (or vitamin H by the Germans - the H stands for Haar und Haut - hair and skin. They really are literal-minded blockheads.)
It can be ingested in supplementary pill form but is found in several natural food sources such as liver, eggs, avocado, some seeds and certain oily fishes.
I would bet good money that Sam has a serious B7 deficiency at best and there's a chance that he's never eaten anything containing it. This would also help explain his triple widow's peak.
