Its the defense you hear from people who have it and are trying to play it down because its never going away. Anyone who says its not a big deal almost certainly has it. The other one is citing many people have OTHER types of herpes in their system so "whats the big deal about this one?"

HPV goes away in its own. Its a mostly transient infection that usually doesn’t even cause cellular changes. It’s not Herpes. If you’ve had sex, you’ve been exposed to HPV - this is how prevalent it is.

An excisional biopsy simply means cutting off tissue. You make it sound like a cure. It does nothing to remove the virus from the host.

Its a curative procedure, as long as the margins are negative.

Having a viral load means nothing as long as the virus doesn’t actually get the the basal layer of the epithelium. So not only does it need to be in you, it also has to find it way to the correct layer.

Good question. It’s possible the person may still have the HPV virus in them. As long as they don’t develop new lesions, then they’re effectively “cured”. Of course people do continue having sex and if they have new sexual partners they will be exposed to other strains as well. But HPV is not like the other taboo having infections. It is so prevalent that if you’ve had sex, you’ve been exposed to it.

For cervical cancer screening, there are algorithms that take into account sexual activity risk.

To summarize, Pap testing with HPV reflex should be done for women 21-29. For those 30 and older, you could do cotesting.

No one under 21 should get a Pap (although there isn’t a physician that isn’t collecting a Pap on this age group) because you shouldn’t treat them, even if they come back with abnormal cells (CIN I) - the lesion is transient and will simply go away. Follow up Pap in 6 months usually shows no abnormal cells.
