It think it's because we are so used to seeing the other (handsome guy) versions of him. Otherwise this would just be good natured Bob you talk to about football at the water cooler.
It's not even that he is handsome, he just always has a pretty lean face. Even roided up as batman, his face was pretty hollow. Chubby cheek Bale is in the uncanny valley.
He's good looking but he's always had an extremely angular face. In American Psycho you can almost see his teeth through his cheeks. It's just weird to see that same guy as a fat man.
