Money matters. The high end plastic surgeons charge a small fortune, but the difference is incredible. But the real key for a natural looking tit job is to have tits in the first place. If a woman has a b-cup already, the implant can go over or under the existing fat for a much more realistic look and feel. The other issue is that instead of enhancing a woman's breast, you're bolting on silicon bags to a boy's chest and there's more to something look physiologically 'correct' than just gluing on body parts to something that shouldn't have them.
