Always seemed like an oversight on Sauron's part that the orc he presumably chose to lead the attack on Minas Tirith had one withered useless arm and tumors on his face obscuring half his vision. Then again the goddamn Witch-king got killed by an old man and a weepy girl.
That was an addition by Peter Jackson, his tendency to go over the top got worse as the trilogy progressed. Just look at the crapshoot that is the Hobbit Trilogy. The orc actually resembles an alien from Peter Jacksons' first horror-gore film: Bad Taste. The withered arm is there because Peter Jackson had to use his mom's oven to bake the silicone makeup and it got messed up so the alien had like one big middle finger.
Well I think what he was going for is having a more experienced looking veteran commander leading the siege. Aesthetically he wanted him to look more battle-worn and as grotesque and "Orcish" as possible. I know people who religiously read the books hated it, but i loved all his portrayals.
